You and your family or other caregivers will receive access to Age at Home’s caregiver website, which has tips and resources often needed for successful independent living. Upon request, Age at Home staff will coach family members who assist in caring for loved ones.
Age at Home caregivers are available to be with you and monitor your condition during your stay in a hospital, physical rehabilitation unit, sub-acute care or skilled-nursing facility, memory-care unit or hospice. This service can be very reassuring and helpful when your family is unable to be with you, or a greater amount of one-on-one companionship is needed.
Age at Home caregivers are available to be in your home 24 hours a day, 7 days a week while providing care from our menu of services.
Caregivers are available to assist client with preparing for bed and any needs throughout the night. This service can be performed on an 8-hour or 12-hour basis.
Caregivers are available to assist you during admission to and/or discharge from a hospital, nursing facility, outpatient clinic or surgery center. A caregiver will accompany you while you are transferring between the facility and your residence, and will stay with you until you are settled.
Age at Home offers personalized support for people with Alzheimer’s, dementia or other cognitive challenges. Our individualized approach ensures that caregivers take into account the client’s life history, routines and preferences.
